All Applicants must appear in person and present: Original Birth Certificate & Identification (such as a valid Driver's License)
Applicants ages 16 and 17 must apply with one biological parent- Long Form Birth Certificate required.
For applicants under age 16, both biological parents must apply- Long Form Birth Certificate required.
Allow 4-6 weeks ( less than 4 weeks requires an expidited fee of $60.00).
