To Appy:
All Applicants must appear in person and present: Original Birth Certificate & Identification (such as a valid Driver's License)
Applicants ages 16 and 17 must apply with one biological parent- Long Form Birth Certificate required.
For applicants under age 16, both biological parents must apply- Long Form Birth Certificate required.
Allow 4-6 weeks ( less than 4 weeks requires an expidited fee of $60.00).

Pasport Book Application Fees:
16 & Over- $110.00 + $25.00 execution Fee = $135.00
Under 16 - $80.00 + $25.00 execution Fee = $105.00
Passport Book Renewal Fees:
$110.00 (in person or by mail)
(Previous passport issued within the last 15 years in your current name
and at least 16 at the time of issue)
Passport Card Application Fees:
16 & over - $30.00 + $25.00 execution Fee = $55.00
Under 16 - $15.00 + $25.00 execution fee = $40.00

Both Passport Book and Card Application Fees:
16 & over - $140.00 + $25.00 execution fee = $165.00
Under 16 - $95.00 + $25.00 execution fee = $120.00
